Your baby is turning one year old—and it's time for their first dental visit! Both the American Dental Association (ADA) and the American Academy of Pediatrics recommend your child first see the dentist around this milestone birthday.
You'll also have a decision to make: do you see your family dentist or a pediatric dentist? While your family dentist can certainly provide quality care for your child, there are also good reasons to see a dentist who specializes in children and teenagers.
The "fear factor." Children are more likely than adults to be anxious about dental visits. But pediatric dentists are highly trained and experienced in relating to children one on one and in clinical techniques that reduce anxiety. Their offices also tend to be "kid-friendly" with bright colors and motifs that appeal to children. Such an atmosphere can be more appealing to children than the more adult environment of a general dentist's office.
The "development factor." Childhood and adolescence are times of rapid physical growth and development, especially for the teeth, gums and jaw structure. A pediatric dentist has extensive knowledge and expertise in this developmental process. They're especially adept at spotting subtle departures from normal growth, such as the early development of a poor bite. If caught early, intervention for emerging bite problems and similar issues could lessen their impact and treatment cost in the future.
Special needs. The same soothing office environment of a pediatric clinic that appeals to children in general could be especially helpful if your child has special needs like autism or ADHD. Some children may also be at risk for an aggressive and destructive form of tooth decay known as early childhood caries (ECC). Pediatric dentists deal with this more commonly than general dentists and are highly trained to prevent and treat this aggressive form of tooth decay.
Seeing a pediatric dentist isn't a "forever" relationship: Once your child enters early adulthood, their care will continue on with a general dentist. But during those early years of rapid development, a pediatric dentist could give your child the insightful care they need to enjoy optimum dental health the rest of their lives.

Proactive dental care is an essential part of childhood growth. But that care can be much harder for children with chronic health issues than for healthier children.
“Chronic condition” is an umbrella term for any permanent and ongoing health issue. Asthma, Down’s syndrome, cystic fibrosis, congenital heart defects and many others fall under this umbrella, with varying symptoms and degrees of intensity. But they all have one common characteristic — a long-term effect on all aspects of a child’s health.
That includes the health of a child’s teeth and gums. Here, then, are a few areas where a chronic health condition could impact dental care and treatment.
Ineffective oral hygiene. Some chronic conditions like autism or hyperactivity disorder (ADHD) that affect behavior or cognitive skills can decrease a child’s ability or willingness to brush or floss; some conditions may also limit their physical ability to perform these tasks. Parents and caregivers may need to seek out tailored training for their child’s needs, or assist them on a regular basis.
Developmental defects. Children with chronic conditions are also more likely to have other developmental problems. For example, a child with Down, Treacher-Collins or Turner syndromes mayÂ be more likely to develop a birth defect called enamel hypoplasia in which not enough tooth enamel develops. Children with this defect must be monitored more closely and frequently for tooth decay.
Special diets and medications. A child with a chronic condition may need to eat different foods at different times as part of their treatment. But different dietary patterns like nutritional shakes or more frequent feedings to boost caloric intake can increase risk for tooth decay. Likewise, children on certain medications may develop lower saliva flow, leading to higher chance of disease. You’ll need to be more alert to the signs of tooth decay if your child is on such a diet or on certain medications, and they may need to see the dentist more often.
While many chronic conditions raise the risk of dental disease, that outcome isn’t inevitable. Working with your dentist and remaining vigilant with good hygiene practices, your special needs child can develop and maintain healthy teeth and gums.

X-ray imaging is a routine part of a child's dental care — and it undeniably makes a difference in preventing and treating dental disease. It's so routine, we can easily forget they're being exposed to an invisible form of electromagnetic radiation.Â And just like other sources of radiation, too much x-ray exposure could increase the risk of cancer.
But while it's possible for your child to be over-exposed to x-rays, it's highly unlikely. That's because healthcare professionals like dentists adhere to a standard known as ALARA when considering and administering x-rays. ALARA is an acronym for “as low as reasonably achievable.” In other words, we only want to expose a patient to the lowest and safest levels of x-ray dosage and frequency that will achieve the most benefit.
To achieve that standard, professional dental organizations advocate the use of x-rays only after a clinical examination of the patient, as well as a thorough review of their medical history for any usage of x-rays for other conditions. If x-rays are warranted, we then take further precautions to protect the patient and staff, and only use the type of x-ray application that's absolutely necessary. For most children that will be a set of two or four bitewing radiographs, which are quite effective for detecting decay in back teeth.
This dosage of radiation in a session of bitewing radiographs is roughly a fifth of the background radiation in the environment a child may be exposed to every day. By spacing these sessions at least six months apart, we're able to achieve a high level of decay detection at a safe and reasonable amount of x-ray exposure.
On top of that, the digital advances in x-ray imaging have reduced the amount of radiation energy needed to achieve the same results as we once did with film. These lower exposure levels and the ALARA standard helps ensure your child's exposure to x-rays will be well within safe limits.

With only a few teeth now showing in your baby’s mouth, you might think it’s too early to schedule their first dental visit. But you should, and here’s why: tooth decay.
Although adults are more likely to contend with dental disease, the exception for children is tooth decay. One kind of decay, early childhood caries (ECC), can wreak havoc in children’s primary teeth. While your child may or may not be at high risk for ECC, it’s better to err on the side of caution and begin regular checkups by their first birthday.
Since primary teeth eventually give way for permanent teeth, it may not seem that important to protect them from decay. But despite their short lifespan primary teeth can have a long-term effect on dental health for one primary reason: They’re placeholders for the permanent teeth that will eventually replace them.
If they’re lost prematurely to decay, nearby teeth can drift into the resulting open space. This can crowd out the intended permanent tooth, which may then erupt out of place (or not at all, remaining impacted within the gums). Protecting primary teeth from decay—or treating them if they do become infected—reduces this risk to the permanent teeth.
Besides regular cleanings, dentists can do other things to protect your child’s teeth from decay. Applying a high strength fluoride solution to teeth can help strengthen enamel against acid attack, the precursor to decay. Sealants on the biting surfaces of teeth deprive bacterial plaque of nooks and crannies to hide, especially in back molars and pre-molars.
You can also help prevent decay in your child’s primary teeth by starting a brushing regimen as soon as teeth start appearing. Also, limit sugar intake by restricting sugary foods to mealtime and not sending a child to bed with a sugary liquid-filled bottle (including juices or breast milk). And avoid possible transfers of oral bacteria from your mouth to theirs by not drinking from the same cup or placing any object in your mouth that might go in theirs.
Tooth decay can have long-term consequences on your child’s dental health. But by working together with your dentist you can help ensure this damaging disease doesn’t damage their teeth.

Keeping your child’s teeth and gums healthy may sometimes seem like “one step forward, two steps back.” You do all the right things like daily brushing and flossing, and keeping sugar consumption to a minimum. But they’re still getting too many cavities.
We can add something else to what you’re already doing to decrease their cavity rate: apply a concentrated fluoride mixture (stronger than what’s found in toothpaste or drinking water) directly to their teeth. Studies have shown that topical fluoride is effective at reducing the risk of new cavities in children at high risk for decay, and may even reverse early decay.
Topical fluoride can be applied as a gel, foam or varnish. The particular method used depends on factors like the child’s age or the preference of the dentist. But any of the three methods can deliver a short-term, high dose of fluoride to the teeth.
As a result, the burst of fluoride strengthens tooth enamel against decay, with plenty of evidence of its effectiveness. As such, the American Dental Association recommends periodic topical fluoride applications for children older than 6, and especially those that appear to be at higher risk for decay.
You might, however, be concerned about the long-term health effects of these stronger concentrations of fluoride. Again, research indicates that the only long-term hazard associated with too much fluoride is a condition called fluorosis, which produces heavy tooth staining. Fluorosis, though, is more of an appearance issue and doesn’t harm the tooth itself. And it can be avoided in the case of topical fluoride by performing the procedure correctly and conservatively.
There’s also a short-term risk of a reaction to the fluoride mixture if the child swallows too much during the procedure, which could cause stomach upset and pain, vomiting or headaches. We can avoid this by using precautions like dental dams and other isolation methods to prevent the child from ingesting it.
Using proper precautions and procedures, topical fluoride is a safe and effective way to give your child added protection against decay. Avoiding this destructive disease can help ensure they’ll enjoy good dental health for the rest of their lives.
